Researchers have shown that improper health care coordination have resulted in declines in functional status especiall y for the chronically ill, unnecessary hospital readmissions and arising avoidable complications.Secondly, as argued by Reisman (2007), there have been failures of health care delivery translating to resources wastes. Some health personnel poorly execute their work. Some of them fail to adopt the best care practices such as patientsà ¢Ã¢â€š ¬ safety best practices and other effective preventive health care practices. Higher costs have been incurred due to this category of waste. It has also led to patient injuries as well as worst clinical outcomes.Complexity in health care administration is another category of waste.
